Licensee Verification

• DPS may reject any application based on the quality of the attached medical card image, the answers provided within the certification or other factors determined during DPS review of a user's submission.
• Refunds are prohibited, regardless of DPS’ decision to accept or reject a submission.
• There is a $2.50 submission cost to process your medical card online.
• Please enter your driver’s license information EXACTLY as it appears on your license. Failure to enter the exact information could cause your application to be denied.
